We at Favia Primary Care (FPC) strive to provide quality medical care to our patients. Our patients have grown accustomed to certain conveniences and we would like to continue such services when possible. In doing so, however, we need to be aware of the increased overhead, resources used, and staff time that these services require. In a desire to continue these conveniences that our patients have grown accustomed to, we have developed Favia Primary Care’s Convenience Club.
Patients and families that decide to become Convenience Club members, will continue to enjoy the below listed conveniences without additional fees. Joining the Convenience Club is not mandatory. For those who decide that membership is not the correct path, they will still have access to the same conveniences, but with additional convenience fees for those services.
Annual membership in Favia Primary Care’s Convenience Club includes several benefits that are listed below. These benefits are available to non-members at an additional fee.
1- Walk In Sick Clinic access at all FPC locations (Mon-Fri 7am-10am) (when clinically appropriate). Acute illnesses only. FPC reserves the right to turn patients away for inappropriate Walk In Sick Clinic visits. FPC reserves the right to limit the number of patients
seen per day. As per insurance terms, visits will be billed through the patient's medical insurance and payment of insurance co-pays are still required at time of service.

2- Telemedicine visit access (when clinically appropriate). As per insurance terms, visits will be billed through the patient's medical insurance and payment of insurance co-pays are still required at time of service.
(Club Members: Included) (Non Members: $25 convenience fee per visit)
3- Blood work drawn prior to a provider visit. Blood draws can be performed at either FPC location. Fee does not include the cost of labs, venipuncture, or lab handling fees. As usual, fees for the actual labs will be billed to the patient’s insurance per insurance terms. Labs drawn at the time of a provider visit and provider directed future lab draw do not require additional fees. Patients requesting lab orders to be provided prior to an appointment at FPC to be drawn at an outside facility will be subject to this fee. This service requires the provider’s time to place the order, staff time to forward orders, and time to retrieve results once available. Patients requesting this service will also be responsible for this fee unless enrolled in Convenience Club Membership.

4- Drawing of blood work ordered by other providers (includes forwarding results to the ordering provider). Convenience Club members are allowed to have outside provider lab orders drawn at FPC when appropriate. Orders must be provided to and reviewed by an FPC provider prior to scheduled blood draw appointment time. The ordering provider is responsible for contacting the patient with results and determining the appropriate course of action. FPC reserves the right to refuse any particular blood draw depending on what is being ordered, the timeline results are needed, and the diagnoses listed on the order. Fee does not include the cost of labs, venipuncture, or lab handling fees.

5- Saturday office hour appointments. Based on availability. Membership does not guarantee availability. As per insurance terms, visits will be billed through the patient's medical insurance and payment of insurance co-pays are still required at time of service.

6- Evening appointments (4pm or later). Based on availability. Membership does not guarantee availability. As per insurance terms, visits will be billed through the patient's medical insurance and payment of insurance co-pays are still required at time of service.

7- Mailing of lab/imaging results to patients by USPS or email. All results are available via portal and/or verbal results via phone call. USPS mailing or email requires an additional fee for non-members.

8- Outside paperwork completion. One to two page paperwork completion outside of scheduled appointment. Paperwork is to be completed when and how deemed appropriate by FPC provider. Includes Department of Motor Vehicle paperwork, employer biometric forms, school medication and sports forms. Disability packets and other forms containing 3 or more pages may require additional fees. FPC does not typically charge for 1-2 page documents pertaining to and presented on the day of an appointment with a provider.

Favia Primary Care Convenience Club Annual Fee:
$125 per individual per year or $225 per family per year
Family inclusion based on medically insured individuals and eligible dependents. Individuals 26
years of age and older are not covered under parents’ membership(s). Members are not restricted in the number of times these services are used per year. FPC reserves the right to limit any one individual’s use of the above services if deemed to be abused or used inappropriately. Any individual restricted for abusive or inappropriate use of these services will not be refunded the membership fee. Any individual discharged from FPC for behaviors deemed inappropriate will not be refunded the membership fee. By joining Favia Primary Convenience Club, an individual agrees to the annual fee along with the above terms and conditions.
I agree to the above terms and conditions and agree to paying the annual membership fee listed above. I understand that the Convenience Club Membership Fee renews automatically on an annual basis and that I am responsible for the payment of this fee. I understand that my membership may be canceled within 30 days of fee payment as long as none of the above services have been provided to me or my included family members. Any request for cancellation of membership must be received by FPC in writing and acknowledged by FPC staff within 30 days of initial membership fee payment or membership renewal payment.
Should any of the above services be provided to me after the payment of my annual fee, I understand that I am not entitled to reimbursement for either complete or partial membership fees paid.
